The Employee Assistance Program (EAP) provides services to help people privately resolve problems that may interfere with work, family, and other important areas of life.  EAP services include counseling, work-life balance, and wellness resources.

As an OEBB member, there is no cost to you or your family members for using EAP services.  In fact, anyone dependent on your income, regardless of age or where they live, and anyone living in your household, related or not, can access these services for free.

If you do not enroll in a 2016-17 healthcare plan by your employer's deadline, you will not have OEBB coverage as of October 1, 2016.

All new Moda Health medical Plans.  New names, new deductibles and maximum out-of-pocket amounts. Please review the plans listed below before open enrollment.

Summary of Medical Plans

**If you are interested in pairing a Health Savings Account (HSA) with your medical plan, you must enroll in Medical Plan Evergreen Synergy.

MODA DENTAL PLANS AND WILLAMETTE DENTAL

Plans 1 - 4, 6, and 8 Willamette Dental Summary of Benefits 

Plans 1,2, and 3 are "incentive plans" meaning as long as you visit the dentist at least once during the year the level of benefit for certain services will increase the following year (up to a maximum of 100 percent).  If you choose a different non incentive plan and then go back to an "incentive plan" your benefit will start over at 70 percent.

MODA VISIONS PLANS

 Vision Plans Opal, Pearl, Quartz, Summary of Benefits

Moda Health Vision Plans Opal, Pearl, and Quartz are structured with a benefit maximum (maximum possible amount the insurance will pay in a given plan year). If you anticipate needing vision services, note the benefit maximum on the  plan you choose and be prepared to pay 100% of any remaining costs after the plan has paid that benefit maximum. All the vision plans have a "frequency schedule" for exams, lenses, and frames, meaning these services are only allowed once within a specified timeframe.  See the Vision Plans Summary of Benefits

HOW MUCH DOES THE DISTRICT CONTRIBUTE?

A CAP is the contribution amount the District will pay each month to OEBB to assist employee in paying for their selected Medical, Dental and Vision Coverage. Currently the CAP is $1245.00 per month.  Employees will pay the remaining out of pocket cost through payroll deduction.
